Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to complete a Travel Questionairre (available from the surgery or via this link Travel Vaccination Form). Once you have returned the completed form to the surgery, the nurse will see what vaccinations you require and we will call you to make an appointment for this and let you know if any charges apply.

The Birches is currently unable to provide the Yellow Fever vaccination. If you require the Yellow Fever vaccination then please contact The Travel Clinic Ltd or Martlesham Health Surgery.

It is important to return this form as early as possible - at least 8 weeks before you travel - as an appointment may be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. 

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
